  • How do I submit my relief records?
    Relief records must be submitted using this submission link: https://app.smartsheet.com/b/form/66321ce6cf4f479184d94499fa66abf0, by no later than the 5th of every month.   • Left over NAFS?
    If the left over NAFS amount is $19.99 or under, those funds may be requested to be transferred to the consumer's personal spending account. If the left over NAFS amount is $20 or over, those funds are required to be sent back to Regional Office.   • I am a Host Home/Relief Home, and I am moving. Who do I need to contact?"
    You will need to contact the Program Manager you are most closely working with, so that they can get a new home inspection scheduled for you, in advance. Additionally, you must inform your direct Admin Central Office contact, to initiate the new home document requirements, put in place by DMH.   • My bank account has changed.  What do I do?
    You will need to complete a new Direct Deposit Form, by one of the following methods below. Download the PDF Direct Deposit Form, here: (https://www.werestorehope.com/forms) Upon completion, please remit completed form and a voided check or bank print out to your direct Admin contact, at Central Office. OR Complete Electronic Direct Deposit Form here: (https://rebrand.ly/Direct-Deposit-Form/electronic). You will be requested to upload a copy of a voided check or bank print out at this link, as well. If you are not able to upload, please remit a copy of your voided check or bank print out to your direct Admin contact, at Central Office.
